HOW TO CHANGE YOUR SURNAME AFTER MARRIAGE in the Philippines

Marriage is surely a very pleasant thing especially for women. But along side with it is the changing of our status, and the name that appears on our IDs. For those who opt not to do it, I have a separate article for this. –> It’s a Choice, Not a Law to Change Your Maiden Name... I have been married since September 2013 but I wasn’t able to change all my ID name until today for some reasons. First, I don’t have the time to do so. My former job is 12 hours a day 6 times a week (whew!). Second, I don’t know how to. It took me lots of researches before I went to a certain government office, since I don’t have the luxury of time, I wanted to make sure that I have everything I need before I go. Third, because I’m still waiting for the expiration of some of my IDs ^_^.

So here it goes:

  1. Philhealth
    • What to bring: Photocopy of Marriage Certificate, bring the original for verification.
    • What to do: Just ask the customer service personnel that you want to change status, fill out the Membership form that she/he will give you. Wait for the processing, and right then and there you will have your status changed and a new ID.
    • Where to go: Any Philhealth office or satellite office, in my case I had it done in their Robinsons-Novaliches branch.
    • How much?: Free ^_^ including the ID, you must have it laminated though.
    • Tip: Since the process will only take 5 to 10 minutes, I suggest you go there before closing time (satellite office closes at 6:00 PM) no more lines, no more waiting ^_^. You have to get a number for other branches.
  2. Bank Records
    • What to bring: Marriage Certificate; they can have it photocopied, valid IDs (they prefer IDs under your married name – this is for BDO and East West Bank; not sure with other banks.)
    • What to do: Ask the teller that you would want to update your records, you would fill out a lot of forms.
    • How much? : Free
    • Tip: If you don’t have your marriage certificate from NSO, certified true copy will do.
  3. Pag-ibig
    • What to bring: Marriage certificate, valid IDs –> Photocopy and original copy for verification.
    • What to do: Tell the guard or any customer service (not yet those in front of the computer) that you will update your status. They will give you a Members Change of Information Form to fill out and a number, wait for your number to be called for your transaction to be processed.
    • Where to go?: Pag-ibig branch near you
    • How much? : Free, unless you want to avail of their Loyalty Card for P100.00 –>Form here<–
    • Tip: The process only takes 5-10 minutes but I would suggest you go there early because the line gets long throughout the day.
  4. SSS
    • *In my case I just filled out Member’s Data Amendment Form (E4) and had it filed by our company’s HR.
    • What to prepare: Filled out form, Photocopy of: Marriage certificate, SSS ID, and authorization letter.
    • How much? : It’s free, but if you will request for a new ID, nowadays it’s called Unified Multi-purpose ID (UMID), ID payment is P300.00 using the Miscellaneous Payment Form (R-6)
    • Tip: The process of changing your status is easy but requesting for a new ID is like how you get it the first time. The only difference is that instead of presenting so many IDs, you only need to present your old SSS ID. Just be sure that you have other valid IDs before you decide to apply for SSSUMID ID because they will confiscate the old SSS ID and you’ll have to wait for 2-3 months for the delivery of the new one.
      • When I was applying for my UMID ID I went to SSS-East Ave. early, but because the process/line from getting the application form to have it verified by SSS personnel (not yet picture-taking) takes too long, I reached the cut off time (4:00PM) of the payment section. The one who verified my application told me that I just have to go back the next day in that branch or in other branch near me.  Since there is a branch here in Commonwealth (new at that time August 2014) I decided to go there but after waiting they said they are not yet accepting payments because their branch is new and that I can pay with accredited banks. I went to so many banks near that branch but to no avail they all said they don’t have SSS receipt and they are only accepting payments every Thursday.  In short I had to go back to SSS-East Ave. The cashier is asking for another piece  of whatsoever paper (I didn’t bother to know what it is) but I insisted that I’m already finished with the verification, so she marked my payment form “non-refundable” but she accepted my payment. I paid my application form, next step picture-taking. I went to that area and saw that it will take me forever and I’m afraid I will reach cut-off again so I tried my luck and went to SSS-Commonwealth. Luckily I was the last one they accepted and holah! I finished my UMID ID application at last! I hope no one will experience this anymore! Got my ID after 2 months!
  5. Passport
    • What to do: Set an appointment at DFA Passport Appointment system
      • Fill out the form properly, print and keep it until your appointment date. (don’t sign it yet; you will do this in front of a DFA personnel)
      • They will not entertain you if you don’t have appointment. (But I noticed that there are still some fixers in the area; I’m not so sure about this though, they have IDs and they’re inside the vicinity of DFA. They are talking to people and I heard they are assisting those who don’t have appointments. They have forms and after some time they were asked to go inside in groups. I don’t know where did they go but definitely they were not merged to those who have prior appointments like us. Again this is just my observation and every body is free to correct me if I’m wrong.)
      • Be on time! There’s a reminder in your appointment letter to come 30 minutes before your schedule. Late comers will have to be rescheduled. My schedule is 10 A.M. but because of heavy traffic, I arrived there at 10:05 A.M. I went inside and show my appointment letter to the guard, luckily the 10 A.M. schedule is still in the waiting area so I was asked to go at the end of the line. There’s a man in our group and he said his appointment is 9:00 A.M. so he was 1 hour late but he was still entertained. So I don’t know how strict they are with the time, but of course this is not a license to be late.
      • Steps:
        • Pre-Step 1: Waiting with your group/batch per appointment schedule to go to Step 1
        • Step 1 : Verification of documents (be sure you have everything you need!)
        • Step 2 : Payment (2nd floor)
          • Express (10 working days) : Php 1,200.00
          • Regular (20 working days) : Php 900.00
        • Step 3 : Picture taking–> Get a number (at your right when you are facing the cashier->specifically for DFA-Aseana), then wait for your number to flash on the screen.
          • Your both ears should be visible, you can smile but you can’t show your teeth ^_^
          • Be sure to check all the data encoded before you sign.
        • Step 4 : Delivery (optional) Php 150.00 delivery fee (location is just after the exit.)
    • What to bring: DFA Documentary Requirements
      • For New application or Change status and name: be sure to bring your original copies, original copies of birth certificate and marriage contract will be left in DFA’s file.
      • For renewal: Photocopy of the front and back page of your passport. (Many didn’t know this part, because it was not written in the DFA website)
      • For those who are changing their Maiden name to Married name you can already use your Married name in setting the appointment.
  6. Driver’s License (I haven’t change my driver’s license yet because I’m still waiting for the expiration ^_^)
    • I guess it’s the same process of renewal, you just have to present your original and photo copy of your marriage contract. It will only take less than one day, if you don’t have any case, if so you’ll have to settle it first at their main office.

    But before anything else, make sure you have secured a copy of your NSO certified Marriage Contract. Wait for over six months after your wedding before requesting for a copy. This is a mistake I would want you to avoid. I requested for the NSO certified copy two months after our wedding just to find out that the marriage is still not registered in the NSO office.   3. Hello. It is very confusing on filling up the application if you wanted to change your surname. I came across your blog but too late for me I have appointment already. What I put in my application is my maiden name. Is it wrong? Would they accept it? Or they will change it to their system when I get their? What should I do?     1. I think it’s okay Ma’am, as long as you bring the necessary documents;
      NSO copy of Marriage Certificate, appointment form, and old passport.
      Don’t forget to have with you a photocopy of the front and back page of your old passport.

    1. Hi Ms. Shirley you only need your old passport, photocopy of first and last page of yoyr old passport and original NSO copy of your marriage contract,it will be left for DFAs record keeping. Have your NSO copy of birth certificate handy but it will not be taken, have it photocopied also just to make sure. I hope it helps. Thanks for dropping by.

        Yes, you need to set an appointment online. Same procedure as renewing your passport. The only difference is you have to bring your original marriage certificate. They will keep it for their record.

    I get my Passport appointment already. I have a doubt because i just only have Voter’s ID wherein i am in single status(using my maiden name) and another POstal ID using my married status(using Husband surname). I would like to chnge my passport into married and use my husband surname. Is it acceptable to use those available valid iDs of mine with different status?

    1. Happy new year Lei, to be sure I suggest you bring your birth certificate and marriage certificate (NSO certified). I wanted to continue using my maiden name unfortunately, insurance companies and banks prefer married names especially when you make your spouse your beneficiary (insurance) or for joint accounts (bank). I do not want to let go of my maiden name so I think a compromise would be to hypenate it with my husband’s last name. Are hypenated surnames acceptable to SSS, Philhealth, Pag-Ibig, BIR, DFA, and LTO? Or do they assign your husband’s surname immediately as your last name? Really appreciate if you can help me on this.

Just for info for all the readers to be on the same boat, hyphenation means you’ll use your surname and will just add hyphen(-) plus your husband’s name. Example: SINGLE: Christine Santos Cruz; MARRIED: Christine Santos Cruz-Lopez (wherein Lopez is your husband’s name).

      According to the law:
      Art. 370 of the new civil code.
      A married woman may use:
      (1) Her maiden first name and surname and add her husband’s surname, or
      (2) Her maiden first name and her husband’s surname or
      (3) Her husband’s full name, but prefixing a word indicating that she is his wife, such as “Mrs.”

      Usually hypehantion is chosen by married woman who doesn’t want to drop their maiden names especially if they are known by their names like doctors, lawyers etc., anybody can use it but there are disadvantages though. I’ve read a lot of post before about hyphenation, others experienced trouble finding their names especially in NBI because for some reasons the computer’s data base are having trouble finding hyphenated names easily. Others experienced being rejected by DFA personnel because not all are familiar with hyphenation. But there are some wh has no problem with it, like for example a testimony of a doctor (I think I read it in femalenetwork.com), she updated her status from single to married for almost all of her records: PRC, Philhealth, passport, and joint bank accounts and had no trouble doing it. According to another source LTO doesn’t allow it.

      I haven’t experienced it but I think trouble comes because it is unusual. As for your question: “Or do they assign your husband’s surname immediately as your last name?” I think if you didn’t explicitly tell them that you want it hyphenated when you update your status and present a marriage contract by default they will assign your husband’s surname.

  8. Hi. I would like to ask if it’s possible to do it the other way around? My IDs are all changed from my maiden to my married name except my voter’s ID. I would like to change my IDs again back to my maiden name. The banks did not accept my application using my maiden’s name because the IDs all have my married name. They did not acknowledge my voter’s ID with my maiden name on it. Thank you.

    1. Hi Marielle, that was an interesting question. I’m not sure with other id’s but for passport once you updated your status and change your surname already you can only revert it of you have a court order to do so.

I renewed my passport many times but keep stating single. Now i want to chane my status from single to married. Wont there be any problem if i renew my passport again?     1. Hi Yie, thank you for sharing your dilemma with us. You have nothing to worry about. Under Art. 370 of the New Civil Code of the Philippines you are allowed to use your maiden name. It’s okay that you used it in your passport even after you got married. Now to use your husband’s name just update your status at DFA. Bring your marriage certificate. After changing your maiden name to your married name though, you will not be able to revert it back in case you change your mind, unless you have a court order. I hope this helps.

  10. Hi, I just got married last October 2016 and until now, our marriage cert has not been in the registry of NSO yet unless I wait for 5 to 6 months. I would like to renew my passport and at the same time, change my status from single to married. Is a CTC of our Marriage Contract from the local civil registar acceptable instead of an NSO copy? And what about my valid IDs, since all of them are still under my maiden name and renewing them takes longer as well. Will these be accepted by the DFA? Thank you for your time.

    1. Hi Ms. Marianne, as far as I know they will accept it. But ofcourse if you’re not in a hurry it wiuld be best to present the NSO authenticated copy. F9r yiyr other IDs I’m sure they will accept the marriage contract from the local civil registrar.

But I provided my marriage contract in french embassy so they can issue me a spouse that allowed me to return to France. Does that mean that my status in the Philippines is married or still single?   12. Hi! I want to renew my passport, and change from my maiden to my married name. I dont have any ids with my married name on it. Will they require me with a married name id? Or will nso marriage contract and birth certificate suffice? On a different note, I updated my sss recently and was told I could not apply for a UMID id, unless I have two Ids with my married name on it. Is that correct? Thank you!

Hi cathy, for passport update your old passport and marriage certificate is enough for e-passport, if yours is brown machine readable then you still need to bring your birth cert, anyhow just to be sure you can bring both. for umid id, if i remember it correctly that was my first ID that was changed so i really don’t have other ids to show. i just brought my marriage certificate.   13. Hi, I just got married last month. I will be claiming my passport tomorrow however, my last name on it was still with my maiden name. How will I change it to my married name? Where should I go? What documents should I bring? I don’t have our marriage contract released by NSO yet. Will marriage certificate from civil registrar work? How long is the process?     Just want to ask how if my mother has a green passport and she wants to change her surname to my fathers surname (married) are they going to issue a new passport i mean the dark maroon color ?
    another is would be Marriage contract from NSO will be fine with other supporting ID’s

      Thank you for checking this blog. Here are the requirements from DFA. Hope it helps. You can get NSO birth certificate online as well.

      Green Passport or passports issued after 01 May 1995:
      Bring original green passport and photocopy of the data page and last page of passport
      Birth Certificate (BC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA, to determine complete middle name
      If PSA Birth Certificate is blurred or unreadable; transcribed certified true copy of the birth certificate from the Local Civil Registry is required when entries in PSA Birth Certificate are blurred or unreadable.
      If born abroad, Report of Birth duly authenticated by PSA

      For women who would change their civil status from single to married:
      Marriage Contract (MC) in Security Paper (SECPA) issued by the Philippine Statistics Authority (PSA) or Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by PSA.
      If entries are blurred or unreadable, submit Transcribed Marriage Contract from the LCR
      If married abroad, Report of Marriage duly authenticated by PSA
      Original and photocopy of Commission of Filipino Overseas (CFO) Guidance and Counseling Certificate of Attendance (required for women with foreign national husband married in the Philippines).

      If you are both emloyed your HR should do it for you, if not you can go to your RDO to update your status. You just have to fill out a form. Bring photocopies of your marriage certificate just in case they will get it.

      As long as the documents were submitted to your registrar then it should be valid and you have a record. Try inquiring at your registrar’s office or directly to NSO.

    I’m a newly wed under civil rights last January 2017 and based from NSO Hotline, I can request my NSO Marriage Certificate after 2-3 months from the date of filing by the City Hall (February 10).
    What’s the difference between the certificate given by the LCR and the NSO?
    Even our HR requires me to submit the NSO copy before they process my request for a new ID.

      For Local Civil Registry (LCR); Civil Registration is a continous, permanent and compulsory recording of vital events occuring in the life of an individual such as birth, marriage and death, and of all court decrees and legal instruments affecting his or her civil status in appropriate registers as mandated by Act of 3753, the Civil Registry Law.

      While National Statistics Office (NSO): The NSO also has responsibility of carrying out and administering the provision of the Civil Registry Law including the archiving of birth, death and marriage and servicing requests for copies and certifications based on these documents as provided for in Act No. 3753 dated February 1931.

      *NSO is the authorized agency to release authenticated copies from all Local Civil Registries.
      *Based on experience if NSO copy is not yet available LCR copy is also accepted.

  20. hi, I plan to apply for UMID and update my status from maiden to married. Am I required to show IDs using my married surname, what if I have none?

    And I thought the UMID is free? I have to pay Php 300 for the UMID or for Update of status and plus UMID?

      What to prepare: Filled out form, Photocopy of: Marriage certificate, and your (old) SSS ID. That’s it, no need to show IDs under your married name.

      The fee is for the new ID (UMID) only, no fee for updating your status.

      Yes I think it’s possible. In my case our HR was the one who updated my Philhealth status from single to married, you just have to present your marriage certificate and maybe an authorization letter stating that you are outside the country and you are authorizing someone on your behalf. Im not so sure which to do first thing,or getting started to change my passport name to my husbands last name. And in LA UNION DFA in Pangasinan do still need to have set an appointment of changing status?

    1. CFO is a requirement of DFA in changing your surname. Get first the CFO in Manila or Cebu branch. You have to set an appointment online for CFO. Also, getting a new passport using your husband’s surname is like renewing your passport so you have to get thru with online application. 🙂

      Actually I think it depends on you, as long as it will not create confusion then I think it’s s okay not to, and vice versa. In my case my driver’s license is the only ID left under my maiden name just because I don’t want to go to LTO main branch. I just had it renewed in the satellite branch.

    We just filed our Certificate of Marriage forwarded to Civil Registry in Baguio(where we got married) this month, but my passport is expiring on Feb 2018. I need to travel on October this year that’s why I need my passport already renewed as early as now. I know I can’t wait for 2-3 months for the NSO copy and you mentioned that they need a certified true copy of the Certificate of Marriage. But we only have one which is our copy. Question is, how do we get a certified true copy of our Certificate of Marriage? Do we still need to go to the civil registry of Baguio to request one? Or any civil registry office would do?

But it’s still an option to call their office to verify or ask someone who is living there if they can be your representative and get a copy on your behalf.         Just updating everyone. Read from another blog that even if the certified true copy of the needed Certificate of Marriage was provided by LCR, DFA will still not accept it for the renewal of passport. So I asked a friend who has an aunt that works in LCR. She said that the document still needs to be authenticated by NSO with a stamp. I’m just not sure if any NSO office will do.

  27. I divorced in American law to my Pilipino husband. But of course in our Philippines Law divorced is not honored to change Last Name to my present Husband. Now in 2016 Dec my Ex husband which is in Philippines Died. I got all the death certificate , our previous marriage contract and my live birth all in red ribbon. Now Question is can I change my last name now to my present husband last name?

    1. Hi Ms. Agnes thank you for stopping by. In the Philippines legally you’re already widowed and I believe you can now revert back to your maiden name, I’m just not sure if you can do it all at once in one processing to change to your new husband’s surname.

    I am recently married.. Last June 2017, I really don’t know what to do with all my ID’s.
    I want to use my maiden name but I think that will cause trouble according to other post.
    But I just want to ask is it possible to change status and surnames in SSS, PAG-IBIG, BIR, PHIL-health and PRC without the marriage certificate from NSO? We only have a copy of certificate from our officiant. Thanks in advance!

      Congratulations on your marriage and welcome to the hustle and bustle of changing your IDs. If you don’t have NSO copy yet as it takes some time for your records to be available there, your civil registry copy will do. 🙂

      Thank you for stopping by. As for my experience you need your valid IDs under your married name if you want to apply for a passport under your married status.

    1. Hi Mercedes, I am not sure if you can do it at the same time, but for lost ID you have to have an affidavit of loss (notarized), then apply for a new one. Bring your marriage certificate just in case it’s okay to change your marital status at the same time. Good luck!

  30. Hi mam.. I am married already but i decided to change my status just recently coz I’m waiting for my passport to expired. My question is if it is ok to present my id consisting my maiden name??   32. tapos na po akong mag change ng passport into married na po, mag renew po ako ng passport, should i change my status to nso or not…yong nso kolang hindi na change.. yong passport ok na..

    1. Hi ma’am Evelyn, thanks for reading this blog. Marriage certificate lang po ang kailangan, hindi naman po chinachange ang birth certificate (NSO, now under PSA). Pero kung married na po ang status nyo sa current passport nyo, I guess no need na po. YUng lumang passport nyo na lang ang kailangan and photocopies ng first and last page, and of course yung schedule nyo po. Thanks again.

    I am not married yet but I had to change my surname into my Mom’s maiden name because when I was processing my passport as I discovered that that was the surname registered in NSO. I didnt have the time to change it into my father’s because it might take me eons to do so plus I got a good opportunity here in UAE which requires an immediate action. How will my update my surname in BIR?

  35. hello po! your blog is so helpful. question lang po regarding sa documents for submission for passport yung original copy ng birth certificate and marriage contract kukunin ba nila? or yung xerox ang kukunin ng dfa? im planning to change my passport na kasi from maiden to married name na kahit next year pa sya mageexpire.. we’ll be applying for a US visa and sabi ng parents namin its better kung married name na ang nasa passport ko. thank you!

I’m applying for a passport for the 1st time, married name na yung nilagay ko kaso ang id lang na meron ako is voter’s id nung single pa ko. napa update ko na kaso wala pa yung updated voter’s id ko. meron din akong marriage and birth certificate. iaaccept na kaya ng dfa yun? or need ko pa ng other supporting documents?     1. Hi Roby, I’m not sure kasi when I applied (renew my passport) I already have my UMID ID chnaged. Pero you can still try kasi may marriage certificate ka naman na. Just be sure na PSA copy na ang dala mo both for birth and marriage certificate. Kukunin nila yang original. Thanks for stopping by. Hope you can update us once para malaman din ng ibang readers.   37. sir/mam ask ko lang po kung mag change status paba ako kasi annuled ako and kaka kasal ko lang ulit so ang mapapalitan lang ba yung name ng wife ko or change muna ako ng single status or apply ko married pero iba lang name ng wife?

  38. my due date is on jan. 2019 and already informed the sss about my pregnancy and filed a maternity notification last june 2018, I got married july 2018 and I already change my civil status in sss. Now I want to request new id in sss with my married name. Is there any problem in my maternity?
